Your child’s teacher handed you the survey and it is very important for you to fill this out. It reflects our school and your voice is truly important. Here at PS217 we aim high, so please help you child’s school get as close as possible to 100% participation rate! However it is most important that you take the time to carefully and accurately answer each question. Your assistance is very much appreciated. All surveys are strictly confidential. Please:
- Return the completed survey in the sealed envelope to your child’s homeroom teacher OR
- Complete the survey online and send in the confirmation receipt.
Every student whose parent completes the survey will receive a one “ONE DAY HOMEWORK PASS” or a “One FREE ICE CREAM COUPON”
